Complete a Miscellaneous/Other Incident Report

1. This tutorial shows you how to add a Miscellaneous/Other Incident Report and how to upload pictures from your device or camera within this report.
2. Click Match Reports
3. Find the relevant Match Report and click the magnifying glass to open the report.
4. Click View Match Report
5. Scroll down to reach the Miscellaneous/Other report.
6. To create a new Miscellaneous/Other report, click the plus sign on the right side of the banner.
7. This form covers a wide range of potential incidents that are not covered by the other report types available. Complete the items that apply and leave non-applicable items blank. Not all questions will apply to every incident.
8. There are four Incident Report Types you can choose from ...
9. Minimum Standards Violation applies to leagues that ask referees to report on compliance violations of the league's minimum standards. Check with your assignor if this report type applies to the league you are officiating.
10. Discrimination accusations refers to accusations of discriminatory remarks based on race, gender, sexual orientation, or any other legally protected characteristic.
11. Other reports may also need to be filed in addition to this report. Be sure to complete all necessary reports to document any cards that were issued, if the match was not completed, or if Ref Abuse/Assault/Gross Mistreatment occurred.
12. Playing Surface/Venue allows you to report an issue with the playing surface or venue. This report is useful if you indicated your "field needs attention" in the Additional Details section of the Match Report. See the example within this tutorial.
13. Other incidents is a catch-all report type when a report is needed for an incident but does not fit anywhere else.
14. For this example, let's complete a Playing Surface/Venue report and add some photos. Select this report type and add a Description.
15. Click Match Status Impacted and choose from one of the dropdown list options.
16. In this example, the Match Status was not impacted, so we will select "no".
If the game was abandoned (that is, was ended), or was suspended (that is, was paused and then resumed), select the appropriate option.
17. Type what you did as the Referee in response to this situation in the Action Taken by Referee section.
18. If one member of the Crew was impacted, select their name from the dropdown list. In this example, no member of the crew was impacted so we will leave it blank.
19. If crew members were impacted in addition to the person you selected, type their names here. In this example, no crew members were impacted so we will leave it blank.
20. Click Select the Team involved in this Incident (if any) to expand the dropdown list.
21. In this example, the team in question was the home team, LSC Select 2.
22. If an offense was committed by any of these parties, select it from the list.
23. Since there was no offense committed by anyone for this report, we will leave it blank.
24. If applicable and you have the name of the person involved in the situation, type their name here.
25. If applicable, indicate when the incident occurred from the dropdown list.
26. In this example, our playing surface/venue issue occurred before the match.
27. Indicate Where the Incident Occurred from the dropdown list.
28. In this example, it occurred on the field.
29. If this report needs input from another official, turn on the Needs Report from other Officials toggle. Click the help text icon to see what turning on this toggle does.
30. Click cancel to close the pop-up window.
31. Let's revise this report to describe a situation where we discovered the goals not being anchored, how we fixed it, and add some photos.
32. Let's also revise the Action Taken by Referee section to describe what we did.
33. We typed in the name of the Assistant Coach here since she helped us fix the goals.
34. No other items apply so we are ready to click Create Incident Report.
35. A summary of our report is now displayed. Click the edit icon if you need to edit the report.
36. Now let's add some photos. Under the Image column, click the number to add images, which is 0 in this case because we haven't added any images yet.
37. You can drop image files in this space, browse and upload files, import them from your device, or use the camera on your phone to upload images.
38. For this example, we will upload photos that are stored on My Device.
39. Select your photo. In this example, it is of an Unanchored soccer goal.
40. Click Open
41. A preview of the photo is displayed. Click Upload 1 file. You can click Add more to select additional photos and upload them all at once.
42. The image now displays in the images list.
43. To add a note to this image, click the edit icon.
44. Type in a Note to describe the image.
44b. Click
45. Click Apply Changes when finished.
46. To use your Camera, click the Camera icon.
47. Click Allow to allow the site to access your camera.
48. Click the camera icon to take a picture.
49. If you don't like the preview of the picture, click Cancel.
50. Let's add another picture from our device showing the fixed soccer goal.
51. Click Anchored soccer goal.jpeg
52. Click Open
53. Click Upload 1 file
54. Click the edit icon to add a note.
55. Type in your Note and Click Apply Changes when finished.
55b. Click
56. Click cancel to return to the report page.
57. We now have two images as part of this other incident report.
